General Information

Services are designed to help elderly, or those with long-term
physical disabilities, to remain as independent as possible
while living in community settings.

Community Options Program (COP) assists individuals with chronic illness or disability to obtain services needed to remain in their own home or to return to their home. COP serves children and adults of all ages with long-term disability. COP services include: home care, meals, transportation, respite care, dental care, equipment, home modifications and remodeling, vehicle lifts, therapy, and counseling.
Waiting lists may exist for Community Options Program funding.

Medicaid Waiver Programs (COP-W, CIP II): Elderly and physically disabled people who are Medicaid-eligible and require a certain level of care may qualify. A variety of services are available through the MA Waiver programs to support the individual in their own home/apartment, Adult Family homes, or Community Based Residential facility. Waiting lists may exist for Medicaid Waiver Program funding.

Medicaid Personal Care Program (MA PCW) assists individuals with personal care tasks such as bathing, dressing, grooming, skin care, transfers, and ambulation. Elderly and disabled individuals who are Medicaid eligible and require a certain level of care may qualify. Medicaid pays for this service, and there is no waiting list for funding for Medicaid Personal Care services.

Elder Adults-at-Risk (formerly Elder Abuse/Neglect): Reports of physical, or financial abuse, or neglect of individuals age 60 or older are investigated. Services may be provided to help eliminate the abuse or neglect and to keep the individual at home whenever possible.

Vulnerable Adults-at Risk: Reports of physical, emotional or financial abuse, or neglect of individuals under the age of 60 are investigated. Services may be provided to help eliminate the abuse of neglect and to keep the individual at home whenever possible.

• Adult Family Home certification
• Supportive Home Care Program services
• SSI-E certification
• Facilitation of Guardianships and Protective Placements
